The low down on Low-T – The Cross Timbers Gazette

Testosterone is the male sex hormone and controls normal sex drive and sperm production. Testosterone also helps maintain bone strength, muscle strength, red blood cell production and physical stamina in men.

As you age, your Testosterone level (T Level) gradually declines (~1% per year after age 30-40). It is important to differentiate between the normal age-related decline in T levels and abnormal drop due to a disease (called hypogonadism or sometimes referred to as Low T). Low T may affect your sexual function and physical well-being.

There are several FDA-approved testosterone treatments which may increase T levels and may help improve Low T symptoms such as low libido, erectile dysfunction and reduced stamina and energy levels.

T treatment should be started after a careful examination and testing by an experienced physician or provider. Sometimes Low T can be from serious underlying conditions such a pituitary tumor, sleep apnea and your physician may order additional testing depending on your symptoms and a thorough physical examination.

If you are noted to have Low T levels, your doctor will probably repeat the test because blood T levels can be pulsatile (variable throughout the day) and therefore the diagnosis should be confirmed before treatment is initiated. In addition if you have underlying conditions that can cause temporary Low T levels (e.g. pain or use of narcotic pain medications or sleep apnea), once the condition improves, these levels will become normal.

Diagnosis & Misdiagnosis

Based on expert guidelines, Low T (hypogonadism) occurs when the blood testosterone levels (drawn before 10 a.m.) are less than 300-325 ng/dL or/and blood free testosterone level (a more accurate test, if a man is overweight or is older than age 65) is less than 55-65 pg/mL. The Low T level should be rechecked at least one more time, before a diagnosis of hypogonadism is confirmed.

Although advertised as an anti-aging formula that improves alertness, mood and stamina, the health benefits of T therapy are not always clear and the performance-benefits (sexual and general vitality) are often short-lived (unless large and risky doses are administered). Unless monitored carefully, T therapy can increase blood pressure, thickness of blood and worsen lipid (fat) levels.

Some older men treated with T therapy in clinical studies, carried out by the National Institute of Health (NIH) experienced increased risk of developing heart problems and strokes.

Treatment

If you have Low T levels, T therapy can improve your quality of life by enhancing your sexual function, improving your mood, alertness, stamina and motivation. If you combine this with an improved diet and increased exercise, it may help shed off some of the belly fat, build muscle mass and improve blood pressure, blood sugar and strengthen your bones. Usually you will start feeling better in 3 or 4 weeks. You should be checked every 3 months in the first year and then every 6 months in most cases. Your prostate gland should be checked annually because in some cases, T therapy may accelerate the normal age related increase in prostate size.

If your Low T level is caused by an underlying condition (such as obesity, sleep apnea, severe pain or serious illness), treating that condition may help improve Low T level.Healthy lifestyle and weight loss may reverse Low T levels in some cases. Starting high doses of T therapy (particularly injections) too early in such cases, can convert a potentially reversible situation into a permanent condition. This is called the testosterone trap. This trap occurs because, when T is given to a patient, his body stops making its own testosterone (suppression of testosterone production by downgrading bodys pituitary to testes communication that normally regulates natural testosterone production in a pulsatile manner). The problem is less likely to occur if the T level is kept in the mid-normal range (usually recommended range is between a serum total testosterone level of 400-700 ng/dL or a free testosterone level of between 65 and 150 pg/mL).

There are other ways to reduce the risk of this testosterone trap. The nasal testosterone preparation, Natesto, may have an advantage over other preparations of testosterone because it is short-acting and is administered three times every day. The blood T levels in patients who take Natesto vary through the day, like that in normal individuals.The risk of testosterone trap is therefore lower.If the older T preparations (such as injections or gels) are used, the risk of testosterone trap can also be lowered by using smaller doses or concomitant and intermittent use of a hormone called HcG (a hormone similar to the pituitary hormone called LH which normally regulates testosterone production by the testes) or by taking a tablet called Clomiphene, a pill which also stimulates the pituitary gland to increase signal directing the body to make more testosterone of its own. These steps may also reduce the risk of side effects of T therapy in most cases.

The number of men using T therapy in the United States has increased dramatically in recent years. Taking this hormone to reverse aging and raising blood levels to youth levels can be dangerous and is not advisable. On the other hand, treatment of legitimately Low T levels can improve quality of life dramatically.It is however important to understand that like any other medication, there are risks and benefits of T therapy. You should undergo a thorough evaluation by your physician before you start T therapy. Only after that should you begin T therapy so you may get maximum benefits of such therapy and at the same time reduce any potential risks.

When is IVF required?IVF procedure is required when either the fallopian tubes are blocked or when the quality of egg or sperm is poor.

The process of IVFSome preliminary blood tests are done to assess the general health and check the ovarian reserve (the number of eggs in the ovary). From the male partner, a sample of the semen is stored as a backup in case of inability to provide a sample on the day of the egg retrieval.

Hormonal injections are given from day 2 of the period for the next 9 days. These injections stimulate ovaries to produce eggs, which are then retrieved under short anaesthesia (15-20 minutes) using transvaginal ultrasound guidance, no cuts or stitches are required.

Embryo transfer is a simple painless procedure, done under ultrasound guidance without anaesthesia.

After the embryo transfer, progesterone is given to sustain the pregnancy.

12 Diets You Should Never Try, According to Health Experts | Eat This Not That – Eat This, Not That

Know how to tell if a diet is right for you? You don't hate doing it. "The one diet to avoid is one that is not sustainable," explains Amy Helms, LMSW, MS, CEDRD-S, RD, LD. These can include diets that cut too many calories or plans that are not "compatible" with your lifestyle. "A plan that is too calorically restrictive will work against you in the long run," she explains. "Our bodies adapt to function on fewer calories, making weight regain just about inevitable."

Additionally, a big cut in calories is a primer for overeating or even binge eating. "For some this may lead to one more failed diet while for others it can lead to disordered eating," she points out. And, while low carbohydrate, high protein, and intermittent fasting methods may be effective in the short-term, they simply aren't sustainable for most people.

While a meat-fueled, no-carb diet may help you lose weight, it can negatively impact your overall health, according to Allen Conrad, BS, DC, CSCSof Montgomery County Chiropractic Center in North Wales, PA. "The Atkins Diet was an idea to include high fat and red meats as a primary component of your meals, and the concept was that you would lose weight this way. The problem with diets like this is that high concentrations of red meat and unsaturated fats can cause many health problems, including heart disease and high cholesterol," he explains. Studies have shown that diets high in red meat and trans fats should be avoided due to their long lasting health concerns.

Keto was the most highly-hyped diet in 2019, but Dr. Conrad isn't a fan. "The concept involves eating low carbohydrate meals with the goal of losing weight. By putting the body into a state of ketosis, the idea is that your body will store less body fat, and you can drop a few pounds," he explains. However, the problem with the ketogenic diet is that this puts additional stress on your internal organs, which need carbs to work, he explains. Additionally, he points to studies that have shown that prolonged low carbohydrate diets were dangerous and could lead to premature death.

The Snake Dieta fad diet comprised of prolonged fasting periods (the initial two fasting periods are 48 hours and 72 hours) with low carbohydrate, high fat meals consumed in between the fasting periodsslithered around social media in 2019, with followers claiming dramatic weight loss results. However, experts hope the diet will shed in popularity in the upcoming year. "The safety and long-term effects are not known as is the case with most fad diets, which are, after all, a fadshort-lived and without scientific basis," says Ania Jastreboff, MD, Ph.D., Yale Medicine endocrinologist and director of the Weight Management & Obesity Prevention.

Juicing supporters boast that it helps cleanse your body of toxins and "build up" from unhealthy food in your body. "What it really is: fruits and veggies that have been stripped of their fiber and packed into a very expensive compostable bottle," explains certified personal trainer and sports nutritionist Holly Roser. "Our bodies are great at cleansing, through our liver and kidneys so the idea of juice removing toxins, is void of scientific backing." If you want to up your fruit and veggie intake, she suggests adding them to your diet in food form. However, if you prefer drinking them, she suggests blending them in a shake or smoothie, "so you're not missing the skin of the fruit or full fiber of the greens."

The Whole30 is meant to help you identify foods that cause you digestive or inflammation issues. Using it to lose weight isn't ideal. For instance, the plan bans legumessomething Roser finds questionable. "Legumes are packed with protein and have zero cholesterol, an amazing alternative to meat. They are an ideal choice to lose weight and live a healthy life with such an impressive nutrition profile," she explains.

And when the diet is overin this case, in just 30 daysyou will likely go back to your old habits. Despite the drawbacks, she does appreciate how the diet encourages people to cook real food instead of consuming processed food. And giving up alcohol is always a good idea. But, "in the end, it's impossible to keep up, like all diets, and people gain weight once they're off it."

Detox is one of those "health" buzzwords that are everywhere these daysfrom detox diets to detox shakes and detox waters. The idea behind "detoxification" is that by following these diets or consuming these products, your body will be cleansed of "toxins," and that in turn will help improve health and promote weight loss, but it's a little more complicated, explains Melissa Nieves, RD, Healthy Meals Supreme. "The concept of detoxification by external means such as diets and detox products is misleading," she explains. "The body itself is detoxifying, every day, at all times. That's what the kidneys and liver are for! In fact, if we were really so full of toxins, we would be hospitalized, not walking around drinking detox shakes!" While the body does detoxify itself, she explains we can help the process by staying hydrated and eating a high fiber diet.

Run, don't walk, away from the HCG diet, a meal plan that consists of just 500 calories a day and supplements or injections of human chorionic gonadotropin (hCG)the hormone that is produced by women during pregnancyseveral times a week. "People do lose weight on this diet, but it's really because they're eating such a small amount of calories a day," points out Nieves. "This type of diet is not sustainable, because who's going to be able to eat such a small amount of food for long periods of time?" Additionally, it's dangerous! Eating so little will lower your metabolism, making it harder to reach a healthy weight in the long-run. It also puts you at risk for malnutrition, can trigger eating disorders, and lead to muscle and tissue loss, "especially in vital organs such as the heart, since the body starts using its protein as energy under starvation conditions." Also, she points out, that the hCG hormone has only been approved as part of fertility treatmentsnot weight loss. "Only small studies have been done so far on the efficacy of using hCG for weight loss, using small sample sizes," she explains. "We don't have enough evidence of the potential health risks of using this hormone, nor whether it really works for weight loss."

Nieves hopes people will grow up when it comes to the Baby Food Diet! "This fad diet is supposed to help you lose weight by cutting calories and controlling portions. It involves replacing one or two meals or snacks a day with baby food. Each jar can range from 20-100 calories," she explains. Again, the reason people lose weight on this diet is due to the small amount of calories consumed each day. But like any other fad diet, it has its drawbacks. These include putting you at risk for malnutrition, "since the nutrient requirements in these foods are specifically set for babies." It is also incredibly difficult to sustain, "since their taste, and the fact that you're not "chewing" your food, is difficult for an adult to get used to," and won't keep you full and satisfied due to their low fiber and protein content. "Also, diets should be pleasurable and practical. It should also help you make and sustain healthy eating habits," she adds. "The Baby Food Diet just doesn't cut it here!"

"This is another diet that cuts out a lot of food groups and restricts your food choices to, well, mostly eggs," points out Nieves. While following this diet, which is very low in carbohydrates and high in protein, can help you shed excess pounds, the results are basically short term. She also points out that it is not a dietary plan to be followed by individuals with diabetes, cholesterol problems, or heart issues.

Eating like a caveman is so B.C.or at least it should be, according to Heather Campbell, MS, RDN, LD, consultant dietitian. "Any diet that requires complete omission or serious restriction of entire food groups like carbohydrates or dairy can be problematic and create opportunities for nutrition deficiencies," she explains. "When your body isn't properly fueled with a balance of all of the needed nutrients, then it will be impossible to create sustainable changes you're looking for." While Keto may lead to initial results, she points out that it's not creating a lifestyle you can continue into the future, "then you're less likely to see permanent positive change in your health."

What do you get when you take the trendy caveman-style Paleo diet and remove almost everything that involves animal products? One of the worst diets of the year, according to health experts. This super restrictive diet only allows things like fruit, nuts, vegetables, seeds, and limited legumes. While it bands all dairy products, you are allowed to eat a tad of meat, so there's that at least. But in general, the super restrictive eating method is difficult to sustain.

If a diet seems too good to be true, it probably is! "Many fad diets don't have peer-reviewed science to back them up at all, like avoiding acidic foods for the Alkaline Diet, or using supplements or drinks to remove toxins from the body," explains Campbell, who points out that people with healthy and functioning organ systems, have an adequate detoxification system already built in. "One way to support your body's natural ability to remove toxins is to focus on healthy habits like drinking enough water, making half of every plate fruits and vegetables, and eating adequate sources of lean protein."

What is testosterone: MD explains all commonly asked questions about the T hormone – T3

One way or another, if you are researching the topic 'how to build muscle', you will come across the question 'what is testosterone' and how different levels of the T hormone can affect the muscle building process. Many will say you need to take supplements if your T levels are low in order to build and maintain muscle mass. But is this true? Is testosterone the key to muscle building?

We asked Marc S. Schneider, M.D., surgeon, medical journal author and co-founder of Dioxyme Craft Nutrition, to answer some of the most frequently asked questions about testosterone. Since T3 is not a medical journal and we understand that people won't reference T3 in their dissertations, we kept the discussion on a level that's informative yet doesn't require extensive knowledge about human anatomy and biology.

As Marc explains, "testosterone is the primary sex hormone found in men and is responsible for androgenic effects (male sexual characteristics) and anabolic effects (growth of tissues). It is responsible for brain and sex organ development in the fetus, the changes that occur in puberty, muscle and bone growth, sex drive and fertility."

Although most people associate high testosterone levels with bullish behaviour, normal testosterone levels are required for a male body to work properly. According to Medical News Today, testosterone "has many important functions, including the development of the bones and muscles, the deepening of the voice, hair growth, and other factors related to appearance [and] the production of sperm"

Low levels of testosterone can have a range of negative health effects, as discussed below.

"Testosterone levels fall within different ranges throughout an individual's life and from day to day. T levels tend to be the highest from ages 16-18 (300-1200 nanograms per deciliter in men) and then drop as the individual enters their 20s and 30s", Marc explains. "As men get older, their testosterone levels may decline about 1 percent per year after age 30", according to Healthline.

Not only the healthy levels of testosterone levels can vary widely in general but also from day to day and right now, the only way to find out T levels is to do a blood test, administered by a medical professional. Despite the broad range in which it's considered healthy, low levels of testosterone can cause health issues in individuals.

Though physicians generally consider this huge range to be normal, as Marc explains, "lower levels within the normal range may leave the individual feeling tired, cause an increase in body fat, decreased mental attention, affect mood, lead to lack of sexual desire or performance issues, and loss of muscle."

On the other hand, it is incredibly rare for there to be excess testosterone without an individual taking an anabolic steroid. "Excess levels are associated with a host of issues including ED, heart disease, liver disease, blood clots, acne, mood swings and impaired judgement", Marc says.

"Low normal testosterone levels are now commonly seen with increased frequency in young and middle aged men", Marc goes on, "probably the most common causes that we see in our clinic in young men is due to a combination of excess stress, long working hours, lack of physical activity, and the constant use and carrying of cellphones in their pocket."

As explained in a Harvard Medical School articled titled Treating low testosterone levels, possible treatments to low-T levels include skin patch, gels and oral therapy, all being non-invasive type of treatment or pellets and injections, the latter two being on the invasive side.

According to Marc, "physicians typically recommend adequate levels of Vitamin D, Zinc, sleep, elimination of stress, exercise (particularly weight lifting/resistance training) and losing body fat, to help reverse lower T levels."

"Though these may support normal higher levels of testosterone, we have not seen this approach alone correct lower testosterone levels", he adds.

Of course, eliminating stress factors is usually easier said than done, but the other factors are well within the reach of most individuals. As Marc explains, "often men will seek to correct the symptoms of low T on their own by taking over the counter supplements. Tribulus terrestris, Tinospora cordifolia, and icariin are commonly claimed to increase testosterone, however clinical studies in humans do not show benefits. Numerous additional herbal remedies are popular yet human data showing that they increase testosterone is lacking."

"The decision to place someone on hormone replacement (testosterone) or prescribe them a testosterone stimulator such as HCG, is dependent upon a combination of their hormonal levels, their overall health, lifestyle and the symptoms they are feeling", he goes on, "the most effective therapy is often a combination of lifestyle modification along with hormone replacement. Combined with an effective exercise and nutritional plan, not only do the individuals improve their energy and sexuality, they can often drop significant amounts of body fat, gain lean muscle and most importantly, feel youthful and vibrant again."

In short, unless it's medically advised, you shouldn't be worry about increasing your testosterone levels with supplements. Most men can benefit from resistance training regularly and following a healthy diet, not just from a T level perspective, but also for improved health, sleep and mood. Increased testosterone levels are just the icing on the cake.

In the last few decades there has been an enormous upsurge in the number of couples facing infertility. This has been attributed to changes in lifestyle, environment and very pertinently the economic and academic aspirations of women, leading to a delay in childbearing. The one constant in the reproductive ability of a woman is her age. Thus, the success of any infertility treatment is majorly dependent on a womans age.Of the many treatments available for infertility, IVF gives the maximum chance of success (30-50%). Be that as it may, simpler treatments like intra-uterine insemination (IUI) and ovulation induction are worth trying if the woman is below 35 years and/or the number of years for which the couple have been trying to conceive is less than three. Whilst these are less intensive the chances of success are around 10-15% per cycle.Though the number of couples using IVF has increased exponentially in recent years, many couples are still reluctant to use it for fear of having an abnormal child, believing that the process of conception is unnatural. These fears are unfounded and precious years are lost by the time they seek IVF. Concerns of physical, financial and emotional stress associated with IVF also prove to be a deterrent. Dealing with infertility can be emotionally taxing so prepare yourself physically and mentally through exercise and counselling and schedule your work so that you are not stressed in any way during IVF treatment.When should a couple opt for IVF and what should they expect during the process?IVF is the only treatment when infertility is due to blocked fallopian tubes or severe male factor infertility. For other cases, a womans age and years of infertility become the deciding factor. In Indian women, ovarian aging occurs earlier than in Caucasian women hence they should get proactive about their treatment once they touch thirty.

What should one expect during the process of IVF? Preliminary blood tests are done to assess general health and check ovarian reserve (number of eggs in the ovary). For the male partner a sample of semen is stored as a back-up in case of inability to provide a sample on the day of egg retrieval. Hormonal injections started from the 2nd day of the period are given for approximately 9 days. These injections stimulate the ovaries to produce eggs, which are then retrieved under a short an aesthesia (15-20 minutes approximately) using transvaginal ultrasound guidance - no cuts or stiches required! The eggs are fertilized with sperm, the resulting embryo is either implanted into the uterus right away or frozen for use later. The injections are not painful though a daily injection may not be very appealing. Egg growth is monitored by ultrasound and a blood test done prior to the final egg maturation injection (HCG). Approximately 4-5 short visits to the clinic would be required - plan your time accordingly. Embryo transfer is a simple, painless procedure, done under ultrasound guidance without an aesthesia. After embryo transfer progesterone is given to sustain the pregnancy. If you don't like injections you can opt for oral or vaginal progesterone. Progesterone supplementation may be increased if your progesterone levels are low. A pregnancy test would be advised after 14 days. These 14 days are the most difficult emotionally as you are agonizing over the outcome. Sharing your fears with a counsellor or a close friend/family member helps reduce anxiety and stress.

A few dos and donts:Bedrest is not required; routine activity improves blood circulation and promotes implantation. Keeping your mind and body active is the key to decreasing stress and improving outcome. Heavy exercise is not recommended.

Travel is not contraindicated however plan it a couple of days after the procedure if you are feeling normal. People respond differently to physical stress some deal with it more easily than others so listen to your body!

An often-asked question is regarding the right diet or a diet that promotes success and the ideal weight. Unfortunately, a change in diet a week or two before the process is not going to change much in your body. Adopting healthy eating habits and regular exercise should be incorporated into your lifestyle. Excessive intake of vitamin supplements does not help. Get your vitamins from natural products like fresh fruit and nuts. Vitamin D deficiency- common in India- impacts fertility so get it checked.

Obesity is a global problem. Unfortunately, SA has not been spared this scourge, and according to the national department of health, about 30% of SA men and a whopping 68% of SA women are overweight. Apart from all the complex causes of obesity, the problem is compounded by the fact that we do not have effective treatments for obesity.

Even drugs that have been approved to treat obesity are only modestly effective, with average weight loss of only 35kg, and they often have horrible side effects. This creates a perfect storm: many people desperate for a solution to their weight struggles, and many willing to exploit this desperation through quackery. One such treatment that is being advertised by weight-loss clinics and spas is hCG (human chorionic gonadotrophin, also known as the pregnancy hormone).

The idea of hCG as a weight-loss treatment originated from the work of Dr Albert Simeons, first described in the medical journal Lancet in 1954, and later in his book Pounds and Inches published in the same year. He recommended daily low-dose hCG injections in combination with a very low calorie diet (VLCD) as a cure for obesity.

Today, the hCG diet, also called the Simeons method or the lipolytic/fat-burning diet, is still being marketed as a route to fantastic weight loss. Advertisements include the promise of extreme weight loss (sometimes up to 1kg a day!), body reshaping to specifically burn abnormal fat such as the tummy roll, appetite suppression and a general feeling of wellbeing and self-confidence. The hCG diet is also endorsed by celebrity doctors.

Companies market a medical weight loss programme that includes a VLCD diet plan, supplements, medical supervision and personal support, along with vaguely labelled injections that upon closer enquiry turn out to be hCG injections. On other sites, one can buy hCG injections and drops, apparently without prescription, along with instructions for the VLCD diet. Products containing hCG can also be ordered on general online shopping platforms.

In one particularly egregious example, a qualified medical doctor with aesthetic medical practices in Johannesburg advertises the Simeons protocol and homeopathic hCG products which, by the definition of how homeopathic products are prepared, cannot contain any active hCG. They also have numerous false claims on their website, including that hCG is present in every human tissue, including males and non-pregnant women as well as pregnant women.

This is simply not true.

The hCG hormone is only produced by cells of a growing embryo during pregnancy, or by hCG-producing tumours, and therefore we do not normally have hCG in our bodies.

It is important to remember that hCG, like all other hormones, has considerable impact on how our bodies function. In women, hCG injections can stimulate ovulation and they form a routine part of fertility treatments. The hormone is also the culprit behind morning sickness during pregnancy.

In men, hCG injections stimulate the production of the male sex hormone testosterone, and as a result, the World Anti-Doping Agency (WADA) has listed hCG as a banned substance in male athletes. Even more worrying, hCG variants present in crude hCG preparations can fuel the growth and spread of cancers.

The name Darcey Silva became synonymous with drunken sobbing, highly impractical outfits, and demands for a marriage proposal. It was outrageous to watch most of the time, but the persona kept Darcey on 90 Day Fiance: Before The 90 Days for three seasons. She even got a spinoff on TLC costarring her twin sister Stacey Silva.

It was during the season finale of Darcey and Stacey that Darcey finally got her proposal. After two failed relationships, a promise band, and an honorary house key, someone gave Darcey a proper ring. And that special someone was Bulgarian fiance Georgi Rusev,who surprised Darcey with a proposal in June.

During filming for the season, Georgi revealed his marital status. He had just begun divorce proceedings from his estranged wife at the time. The process is ongoing, so Darcey will have to wait to walk down the aisle. But that doesnt mean she cant start planning ahead.

Will the wedding be filmed for TLC? You bet it will. Will Darcey want to look her best for the cameras? You bet she will. Thats why Darcey is taking an extreme route to lose weight for her big day. Newlywed sister Stacey is joining her on the regime.

Soapdirt reported that Darcey and Stacey are currently on a low calorie diet. They are also on a HCG program for weight loss. HCG involves injections of Human Chorionic Gonadotropin (HCG) for a 45 day period.

The unconventional method seems to be working for Darcey. She supposedly already lost five pounds.

Darcey estimates that Georgis divorce will take a few more months to settle, so the couple are planning to wed sometime next year. Darcey revealed that it will be a beach destination, though a definite location hasnt been chosen yet.

The wedding, whenever it happens, will be another marketing opportunity for the twins clothing line House of Eleven, should the bride decide to wear one of her designs for the event.
