Weight-loss surgery may release toxic compounds from fat into the bloodstream – The Hub at Johns Hopkins

Toxic man-made chemicals that are absorbed into the body and stored in fat may be released into the bloodstream during the rapid fat loss that follows bariatric surgery, according to a study from researchers at the Johns Hopkins Bloomberg School of Public Health. The finding points to the need for further research to understand the health effects of this potential toxicant exposure.

For the study, published online in Obesity, researchers examined 26 people undergoing bariatric weight-loss surgery and found evidence of post-surgery rises in the bloodstream levels of environmental toxicants that are known to be stored long term in fat, including polychlorinated biphenyls, organochlorine pesticides, and polybrominated diphenyl ethers. Polychlorinated biphenyls, or PCBs, are a class of chemicals previously used in plastics, dyes, and electrical equipment. Although no longer commercially available, these toxins have been linked to cancers and disruption of the immune, reproductive, nervous, and endocrine systems, according to the Environmental Protection Agency. Organochlorine pesticides have been shown to act as endocrine disrupting chemicals and the effects have been linked with hypertension and cardiovascular disorders. Polybrominated diphenyl ethers are PCB-like chemicals that can be inhaled or ingested, and although not directly linked to cancers, these toxins have been linked to the development of tumors in rat and mouse studies. Bans on the U.S. manufacture of PCBs and the organochlorine pesticide DDT took effect in 1979 and 1973, respectively.

"The fact that this increasingly popular type of surgery may be causing these compounds to be released into the bloodstream really challenges us to understand the potential health consequences," says study senior author John Groopman, a professor in the Bloomberg School.

About 16 million people in the U.S. are morbidly obese, defined as having a body mass index of 35 kg/m2. Morbid obesity confers a relatively high risk of type 2 diabetes, cardiovascular disease, hypertension, and many cancers. For almost three decades, the U.S. National Institutes of Health has recommended weight-loss surgeries called bariatric surgeriesincluding stomach stapling and gastric bypass proceduresfor people who are morbidly obese and have serious obesity-related conditions such as diabetes, as well as for anyone with a BMI over 40. More than 200,000 bariatric surgeries are now performed in the country every year.

Although bariatric surgeries are known to have potential adverse side effects, including micronutrient deficiencies, the release of environmental toxicants from fat storage isn't one that has been studied before. Recently, though, Groopman and other researchers from the Bloomberg School and the Johns Hopkins School of Medicine realized that as these surgeries were becoming common, and, since they involved rapid fat loss, they could potentially release toxicants stored in fat into the bloodstream of patients.

The researchers enrolled 27 patients who were morbidly obese and scheduled for surgery at the Johns Hopkins Center for Bariatric Surgery. Of these, 26 patients went through with the surgery and were included in the analysis. Each patient had a blood sample taken at the time of surgery and on four follow-up visits over six months. The analysis, aided by experts at the U.S. Centers for Disease Control and Prevention, focused on the bloodstream levels of 41 toxicants, including 24 PCB compounds and five organochlorine pesticides that were detectable in the initial samples. The researchers also classified the patients into two groups: those born before 1976, when most of these chemical compounds were still widely used, and those born after. The first group contained 17 people and the second group contained nine.

The results showed that the patients born before 1976 started the study with roughly 5 times higher serum levels of most of the PCBs, as well as higher levels of organochlorine pesticides, compared to the patients born after 1976. During the six months of weight loss after surgeryin which the patients lost an average of 23 percent of their initial weight, or nearly 30 kg eachboth groups of patients tended to show increases in serum levels of fat-stored toxicants. For example, in the older patients, serum levels of most PCBs went up by 15 to 25 percent for every 10 kg of lost weight. The older patients, who started with higher baseline serum levels of most toxicants, and presumably had much greater fat stores of these compounds, tended to have higher rates of increases in serum levels per unit of lost weight, especially for PCBs.

The researchers note that studies with larger populations need to be done to verify the findings in this small-sample study. Further research, they add, could also determine if there are any biological effects, especially on sensitive, fat-rich organs such as the liver and brain, of the toxicant doses involved.

"There is remarkably little information in the literature that relates blood levels of these compounds to effects in tissues," says Groopman. "It would be great if we determine that it's not a problem, and it might turn out not to be. At the same time, the data might suggest that people should lose weight more slowly, or that we should somehow find ways to trap these compounds as they are released into the blood. We also need to recognize that the compounds released from the fat represent lifetime accumulation of toxicants, and we need to address these life course exposures to determine health risk."

Obese teens’ quality of life improves after weight loss surgery – Reuters

(Reuters Health) - Obese teens who have surgery to lose weight report better quality of life, less joint pain and fewer physical limitations after their operations than before, a new study suggests.

Researchers followed 242 obese teens who had so-called metabolic and bariatric surgery to lose weight over three years, starting when they were 17 years old, on average. Prior to surgery, half of the teens had a body mass index (BMI) of at least 50, making them severely obese; two-thirds had musculoskeletal pain and joint pain and half had poor physical function.

Three years after surgery, the teens average reduction in BMI was 27%, which meant they were still obese. However, previous research has shown that a BMI reduction of just 5% to 10% is enough to result in meaningful improvements in health and quality of life.

And in the current study, researchers found that every 10% reduction in BMI was associated with 6% lower odds of the teens having musculoskeletal pain and 10% lower odds of having joint pain.

Metabolic and bariatric surgery leads to large and sustained reductions in joint pain and improvements in physical function in adolescents with severe obesity over 3 years, Dr. Sharon Bout-Tabaku of Nationwide Childrens Hospital in Columbus, Ohio, and colleagues write in Pediatrics.

These improvements will allow teenagers to move, be more functional, and participate in physical activity to improve their joint health and maintain their weight loss, the study team notes.

More than one in five American teens are obese, according to the U.S. Centers for Disease Control and Prevention (CDC). Teens are considered obese when their BMI is at or above the 95th percentile for youth of the same age and sex.

Adolescent obesity is associated with chronic joint pain and with irreversible joint damage, the researchers note. But even as a growing number of obese teens get metabolic and bariatric surgery to lose weight, research to date hasnt offered a clear picture of how these operations impact future joint health.

In the current study, the decline in BMI wasnt directly associated with changes in poor physical function three years after surgery.

But young people who complained of joint pain after the surgery were 21% more likely than those who didnt to also report poor physical function.

The study wasnt designed to prove whether or how surgical weight loss might directly improve joint pain or musculoskeletal pain in obese teens.

One limitation of the study is that the data came from a research project designed to study improvements in weight and obesity-related disease, not joint pain, the study authors note.

Another drawback is that data on physical improvements was based on patient surveys, not on objective measures of physical function or body mechanics.

Even so, the results suggest that improved joint health can be added to the list of potential health benefits of weight loss surgery, the authors conclude.

Adolescence represents a window of opportunity for caregivers to implement exercise and behavioral supports pre-and post-surgery to maintain long-term weight and joint health benefits, the study team writes. Future research should be focused on studying the relationship among weight loss, biomechanical and systemic inflammatory mechanisms, performance measures, and biomarkers to identify, target, and treat adolescents who are obese and at risk for knee osteoarthritis.

Hoda Kotb is so wonderfully relatable when it comes to the struggle of trying to eat better.

During Monday's fourth hour of the Today show, Hoda and her fellow co-host Jenna Bush Hager stepped on a scale while announcing their intent to try out intermittent fasting, a popular weight-loss diet among celebrities that asks you to restrict when and how much you eat during a specific period of time. The pros and cons of the plan have been hotly debated in the nutrition world, but if there's one thing everyone can agree on, it's that you should consult with your doctor first before giving it a go.

To launch their experiment, Hoda and Jenna tried out each other's scales to make sure the numbers they were seeing were accurate. Hoda's weight was recorded at 158 pounds, while Jenna's scale read 171 pounds.

A day after revealing her weight on the show, Hoda posted a video of herself sweating it out on a stair machine at the gym. In that same post, though, she also shared a picture of an Oreo cookie and wrote in the caption, "Me yesterday: HEY: 158 lbs ... I am coming for you! Me after workout: swipe [to the cookie picture]."

To show solidarity, Hoda's fans penned encouraging and hilarious words to the NBC morning show anchor. "Im with ya, girl!! " one fan wrote. "I worked out yesterday and came home to eat 4 cookies . Thats a balanced diet, right?" another joked.

Meanwhile, Jenna (who just gave birth to her son in August) hasn't posted anything on Instagram about her progress just yet. With that being said though, the mother-of-three spoke up about Monday's weigh in on Tuesday and clarified that she's doing this more to "be healthy" instead of to lose weight.

"I was reminded why I don't weigh myself yesterday because I was playing with my kids, and I kept having that one number in my head," Jenna explained. "And I'm like, 'You know what? No, no, no, no.' I don't want life to be controlled by a scale."

Whatever happens, we wish Jenna and Hoda the best of luck in their quest.

Most people have negative self-talk. Thiswave of gloom that threatens to block out all the blue skies and all the silver linings. For Bilawal Khan, the thunder was two-fold; it came from himself and it came from everyone he knew. The constant refrain of lose that weight was tough.

The 24-year-old Pakistani, who has been in the UAE since he was a year old, says he used to love sweet things sugary drinks, chocolate, and so on. And no amount of playing field games such as football could balance out his intake. He kept packing on the kilos.

It wasnt only Khans self-esteem that was taking a hit though; it was also his sleep. As Khan would fall into a deep slumber each night, he would begin to snore. And snore. And snore. The sound was so jarring and so loud that his parents with whom he lives in Dubai would wake him up just to ask him to stop. But snoring is caused by the relaxing of throat muscles and the interaction of airwaves the more disturbed the breathing, the more sonorous it would seem. Each time the Khan would fall asleep, he would relax and so would begin the disturbing rhythms.

Tired of all the backlash, from friends and family, the 5-foot-2-inch tall expatdecided on a course of action once hed hit 108kg. He joined a gym. And he decided to rid himself of sugar drinks, at least for the moment. It was in November 1, 2018. The first phase was obviously very tough. In the gym, people were looking at me and saying, what he can do? My coach said, now they are making fun of you but there will be a time they will appreciate you.

Food for the first three months

"For the first three months, I used to put lemon and honey in hot water and I used to drink that that was my breakfast I used to only drink that. In the afternoon, I would have peanut butter and two slices of brown bread. At night I used to have salads," recalls Khan.

When he joined, Khan used to do 1.5hours cardio and 30 mins of high-intensity interval training. He used to shy away from people. I used to call my cousin and talk to him at the gym. I used to say, 'people are laughing, so I dont feel comfortable'. He said, just keep calm, use that as a motivation, dont take it in a bad way and one day you should tell them, see, one day you laughed at me, and you look at the results.

And its worked, exclaims Khan, over the phone. There was a time those people [who laughed at me] were appreciating me - on the last day when I was leaving my old gym, they said, you are the motivation for us.

Today, the sprightly 79kg part-time student, part-time working man says whats left of his long journey is stretch marks, a wariness of junk and sugary treats, and a built-up self-confidence thats not going anywhere anytime soon.

Now Im gaining [muscles], Im doing weight-lifting in the gym, says Khan. Now, I eat chicken breast,red beans and all the items which have protein in it. Its mostly boiled food, he says. But thats ok. Its a small price to pay to still the screaming voices. To quiet the negative self-talk.
