Low-FODMAP diet catches on among people with digestive misery – USA TODAY

Would a life without onions, garlic, apples and most bread be worth living?

For growing numbers of people suffering from digestive ills, the answer is "yes,"at least for a while.

They are trying something called the low-FODMAP diet, an experimental eating plan developed in Australia and now getting some scientific backing and commercial buzz in the United States. The diet is aimed primarily at people with irritable bowel syndrome, a common condition in which abdominal pain and irregular bowel habits with diarrhea, constipation or both can wax and wane for years.

Followersare asked to eliminate, and then try to reintroduce, a long list of foods containing certain carbohydrates that might trigger gas, bloating, pain and diarrhea. FODMAP is an acronym for those carbohydrates: fermentable oligosaccharides, disaccharides, monosaccharides and polyols.

The theory is that FODMAPs, found in many fruits, vegetables, grains, dairy foods and sweeteners, are poorly digested by susceptible people. The undigested carbohydrates can draw extra fluid into the digestive tract and end up in the colon, where they are feasted upon and fermented by gut microbes, leading to all sorts of misery, says Lauren Van Dam, a registered dietitian at the University of Michigan.

If giving up some foods might lessen that misery, many people are game, she says. We have some patients that are literally housebound because they are worried they are going to have an accident or because they have terrible pain, Van Dam says.

Barbara DeLancey, a 63-year-old registered nurse from Ann Arbor, says she was crippled by 10-times-a-day diarrhea when she agreed to try the diet a year ago.I was resistant at first because it seemed so restrictive, she says. She got teary-eyed at the grocery store, seeing all the food she could not have.

But eventually shelearned she could tolerate and add back some favorites, including garlic and apples. She credits the diet, along with medication, with a big reduction in her symptoms, but says it has not been a miracle cure.

The diet has generated enough interest to prompt some food companies to introduce low-FODMAP products hoping for a market surpassing that for gluten-free foods, according to some industry reports.

In 2014,IBS treatment guidelinesfromthe American College of Gastroenterologysaid the diet showed promise, but that the quality of evidence was very low."

Since then, several studies have strengthened the case for the diet, says guideline co-author Brian Lacy, a professor of medicine and chief of gastroenterology at Dartmouth-Hitchcock Medical Center, Lebanon, N.H. Still, one study from Sweden showed no advantage over a less-restrictive dietary approach that eliminates fewer foods and stresses changes such as smaller, more frequent meals.

"The data are not overwhelming, Lacy says. But I think its a reasonable thing to try.

The largest study so far, with 92patients, was conducted at University of Michigan.It found that 52% of low-FODMAP patients and 41% of patients on a less-restrictive diet reported adequate relief.The difference was not statistically significant, says study co-authorWilliam Chey, a professor of gastroenterology. But low-FODMAP patients did get significantly better relief from pain and bloating and greater gains in mood, body image and overall quality of life, he says.

Medications, probiotics, fiber supplements and other approaches still have roles in treating IBS, and no one approach works for all patients, Chey and Lacy say.

Its not known if the diet leads to any nutritional deficiencies or keeps working in the long run especially for patients who get little support from doctors or dietitians.

Physicians are largely administering the diet by giving patients sheets of paper with foods to exclude, Chey says. Ideally, all would be referred to dietitians, Chey and Lacy agree. But few dietitians are trained in the diet, and insurers typically do not cover the visits, Lacy notes.

When patients do make their way to Kate Scarlata, a Boston area registered dietitian who specializes in the diet, the first thing she tells them is that it is a dietary experiment. Its a short-term learning diet.

Van Dam does the same explaining that after a few weeks she will help clients reintroduce foods one carbohydrate category at a time.

Most are able to identify some high-FODMAP foods they can tolerate very well and some they can tolerate a little, but not in high portions every day, she says. Our goal is for them to be on the most varied, nutritious diet possible.

The trickiest part is avoiding onions and garlic, the dietitians say. They teach clients strategies such as using (safe) garlic-infused oils,avoiding restaurant sauces and soupsand topping salads with oil and vinegar.

Mapping FODMAP

High- and low-FODMAP food lists are updated frequently at websites and apps run by the University of Michigan and Monash University in Australia.

But as one nutritionist has revealed, some of the most well-known and trendiest diets could actually pose a danger to your health.Registered nutritionist Rhiannon Lambert has unveiled the fad diets that you should never follow if you want to stay healthy, from the popular juice cleanse to becoming a vegan on a whim.

Juice cleanse

Any suggestion that the human body can be detoxed with a juice cleanse is incorrect. We are naturally designed to remove toxins using our liver and kidneys a juice cleanse won't perform such a detox.

Those who promote juicing often claim that drinking juice is better than eating whole fruits and vegetables because removing fibre makes nutrients easier to absorb. However, there is no scientific research to support this claim.

Antioxidants are often bound to the fibre in fruit and vegetables and countless research studies suggest that the lack of fibre in juices significantly reduces any supposed health benefits.

Consuming well under 1,000 calories per day from juices will likely result in a stark calorie deficit and weight-loss will be inevitable. But this is extremely difficult to sustain for more than a few days and while a juice diet may help initially with slimming, any severe calorie restriction can slow down your metabolism long-term.

Juices should not be used as meal replacements. They do not contain sufficient protein nor healthy fat - both of which are required by the body. In fact, drinking juice over a long period of time could increase the risk of metabolic syndrome, liver damage and even obesity.

Veganism

At this time of year, many people try veganism to become healthier and lose weight.But only by safely applying a vegan diet will you avoid deficiency in key nutrients.

There are some nutrients that can only be obtained from plants such as vitamin C, and others that can only be obtained from animal sources such as Vitamin B12, which is involved in the function of every cell in the body and brain.As it isn't found in any non-animal product (except algae), studies suggest vegans are more often than not deficient in this critical nutrient. Other nutrients often forgotten in a vegan diet include vitamin D, omega-3 and iron.

Protein is of course vital for muscle and bone health but also for cells affecting our skin and hair. With about 20 per cent of the human body made up of protein, its important to get an adequate amount from your diet every single day. Thankfully, there are plenty of delicious, protein-rich plant-based foods to consider incorporating into your diet, including tofu, lentils, quinoa, hemp, chia and beans.

Alkaline diet

This weight loss scheme is based on the claim that modern diets cause our body to produce too much acid, which is then turned into fat, leading to weight gain. High acidity levels have also been blamed on conditions such as arthritis, osteoporosis, tiredness, and kidney and liver disorders.

An alkaline diet reduces the number of acid-producing foods you eat, such as meat, grains, refined sugar, dairy, caffeine, alcohol and processed foods, in favour of alkaline foods.Ultimately, the diet will consist of a great deal of fruit and vegetables and a small amount of protein.

While the advice to cut back on processed foods, alcohol, caffeine and refined sugar is good, any weight loss associated with this diet is largely down to the fact these unhealthy foods have been cut out, rather than anything to do with acid and alkaline.

The human body regulates its own acidity levels, regardless of what you eat and drink.

Unlike so many other diets, the alkaline diet is not unhealthy, but its claims are simply not supported by science and human physiology.

Meal replacements

The idea is to restrict calories by eating a range of bars, soups, and shakes, as well as having a few low-calorie meals.

Like with any calorie-restricting diet, weight loss can be dramatic. Despite some products being fortified with vitamins and minerals to ensurenutritionisn't lost, the very mindset of giving up normal meals and swapping them for a snack bar or a shake can be boring, socially isolating and difficult to maintain long-term.

And while fortified products are a welcome addition to any restrictive diet, the inclusion of sugar and artificial chemicals is definitely not, with studies suggesting that they will limit any health benefits from vitamins and minerals.

A blackcurrent and apple shake, for example, contains 44.1g of sugar per 100g significantly above your recommended daily intake.

Low calory meal plans

Low-calorie weight loss programmes are based on your Body Mass Index (BMI) score, which will dictate how many meals a day you have.

This diet does advise you to see a GP before you start and can fit into a busy schedule - but it has many drawbacks. Restricting carbohydrates and fibre will result in side effects such as tiredness, dizziness, and constipation, making this plan unsustainable long-term.

It could also impact on your mental well-being. Without any allowance for spontaneous eating out, they are unsustainable as diet plans.Any low-calorie diet that involves eating 1,000 calories a day or less should not be followed for more than 12 continuous weeks and should require professional medical supervision.

Low fat meal plans

These diets encourage the swapping of high-fat foods for low-fat foods which are naturally filling. But this idea is out-dated, with healthy fats now considered a crucial element in our meals.

On these diets, some items are considered 'free foods' which can be eaten in unlimited amounts including fruit, vegetables, pasta, meat, fish and eggs. No calorie counting is involved and no foods are banned, and it's said you could lose one or two pounds a week.But without any education surrounding portion sizes and nutritional intake, many will find it difficult to understand what to do when the programme ends and struggle to keep the weight off long-term.

There would also be a temptation to fill up on 'free foods' such as processed meats without any concern for the salt, sugar and other unhealthy additives contained in them.With many carbohydrates restricted, these diets will only worsen some people's obsessive fear of supposed 'bad foods'.

You might be focused on outcomes when you exercise: stronger muscles, weight loss, or other aspects of better health and wellness. But if you don't focus on the exercise, and what it takes to do it safely, you may set yourself up for workout injuries.

Safe exercise requires planning and careful execution. Start by finding the best exercise for your ability. For example, if you have joint pain, you can avoid workout injuries by choosing exercise that relieves joint pressure, such as swimming or cycling. If you have balance problems, a supervised exercise program with a personal trainer might be a safer bet. Discuss the options with your doctor, a personal trainer, or a friend; and get the okay from your doctor before starting a program, especially if you have heart or lung disease.

Getting the right equipment also helps ensure safe exercise. If using hand weights, start with a level that matches your current ability. And choose clothes and shoes designed for your type of exercise. For example, wear reflective clothing if you're going to walk, run, or cycle outside, so you'll be visible to drivers.

While you're exercising, avoid workout injuries by following some simple guidelines. Make sure that you:

Cool down after a workout for five to 10 minutes. It won't prevent workout injuries, but it will help prevent muscle cramps and dizziness while slowing quick breathing and a fast-beating heart. An easy way to cool down is to walk around or gently march in place until you notice that your heart rate and breathing are slowing down.

In between workouts, make sure you pay attention to muscle soreness. Soreness that begins 12 to 24 hours after exercise is normal. But if you have persistent or intense muscle pain that starts during a workout or right afterward, or persists more than one to two weeks, call your doctor.

Also, avoid training too hard or too often. Either can cause workout injuries from overuse like stress fractures, stiff or sore joints and muscles, and inflamed tendons and ligaments.

If you go for weeks or months without exercising, drop back if necessary when you start again by shortening cardio workouts, doing cardio at a lower intensity, lifting lighter weights, choosing easier exercises, or doing fewer reps or sets. This will help keep you on a safe exercise program and avoid workout injuries, so you can keep exercising in the future.

The concern with fast weight loss is that it usually takes extraordinary efforts in diet and exercise -- efforts that could be unhealthy and that you probably can't maintain as permanent lifestyle changes.

A weight loss of 1 to 2 pounds a week is the typical recommendation. Although that may seem like a slow pace for weight loss, it's more likely to help you maintain your weight loss for the long term. Remember that 1 pound (0.45 kilogram) of fat contains 3,500 calories. So to lose 1 pound a week, you need to burn 500 more calories than you eat each day (500 calories x 7 days = 3,500 calories).

Also, if you lose a lot of weight very quickly, you may not lose as much fat as you would with a more modest rate of weight loss. Instead, you might lose water weight or even lean tissue, since it's hard to burn that many fat calories in a short period.

In some situations, however, faster weight loss can be safe if it's done the right way. For example, doctors might prescribe very low calorie diets for rapid weight loss if obesity is causing serious health problems. But an extreme diet like this requires medical supervision. And it can be difficult to keep this weight off.

In addition, some diets include an initiation phase to help you jump-start your weight loss. You can lose weight quickly with an approach like this because it combines many healthy and safe strategies at once -- no gimmicks or extreme dieting. After the initial two-week period, you transition into the recommended weight loss of 1 or 2 pounds a week, which is not only safe but also realistic and sustainable for the long term.
