A Dermatologist Is Creating Unnecessary Panic Around Spironolactone and COVID-19 – VICE

If you follow enough trans women on Twitter, you might have seen a panic break out earlier this week over concerns that spironolactone, a testosterone blocker that many of us take as part of hormone replacement therapy that is also widely prescribed to the general population to treat hormonal acne, increases ones risk for developing COVID-19. Thankfully, according to the medical experts VICE spoke with, there is absolutely no cause for concern.

The mess, like many before it, all began on Instagram. Ellen Marmur, a New York City dermatologist, posted a video to her feed on March 21 in which she recommends that her patients stop taking spironolactone, also called spiro. The potassium-sparing diuretic was initially developed in 1957 to treat hypertension and congestive heart failure.

I would stop spironolactone, Marmur says in the video. It is not medically necessary for acne. I am only talking about my acne patients. I would stop the spironolactone until we know further.

As Marmur explains in the video, her recommendation is based on her own research into spironolactones effect on angiotensin converting enzyme 2 (ACE2) receptors, which appear to serve as one possible entry point for SARS-CoV-2, the novel coronavirus that can cause COVID-19. This is not a recommendation shared by the American Academy of Dermatology, she notes at the top of the clip; she merely speaks for herself. A number of outlets ran with the recommendation shortly after Marmur posted the video to her Instagram account, which has nearly 16,000 followers at press time. Pop Sugar and RealSelf told readers that they might want to stop taking spiro as a precautionary measure against COVID-19. Neither site makes note of the key role spiro plays in trans healthcare, nor does Marmur in her Instagram video.

In the days that followed, an increasing number of trans people started sharing the RealSelf and Pop Sugar articles on social media, anxiously wondering whether a medication that many consider to be life-saving may actually be putting their lives at risk. The concern is more than valid. Trans healthcare and its impact on actual trans people has been woefully under-researched since its inception, leading many trans individuals to trust anecdotal information from other community members as much as if not more than the recommendations they get directly from providers. Given our constantly evolving understanding of the coronavirus pandemic, not to mention spiros negative reputation in many pockets of the community, we would have every reason to worry after reading RealSelf or Pop Sugars alarming and, frankly, irresponsible reporting.

VICE spoke with medical providers who oversee trans healthcare and other services at two LGBTQ community health centers, both of whom agreed that there is no reason to believe that taking spiro will make any individual, trans or cis, any more susceptible to COVID-19. The data is still inconclusive regarding the role of ACE2 receptors and coronavirus susceptibility, said Julie Thompson, Medical Director of Trans Health at Fenway Health in Boston. Asa Radix, Senior Director of Research and Education at Callen-Lorde Community Health Center in New York City, echoed Thompsons sentiments: I would not want someone to stop the medication based on no evidence.

The argument that spiro might make one more vulnerable to the virus may appear to be scientifically sound at first, with many credible citations to back it up. But in the words of Cher Horowitz, its a full-on Monet: From far away its OK, but up close? Its a big old mess. Marmurs claim likely draws on a study from 2005 about how spiro increases the activity and expression of your cells ACE2 receptors, which, as discussed earlier, are believed to be key to the novel coronavirus replication process. (Radix was skeptical about this study during our interview, noting that it only involved 10 patients.) Using a kind of transitive logic, the people making the case against spiro then connect that studys findings to current research into whether medications that affect ACE2 receptors might increase risk for COVID-19 infection. In other words, if A equals B and B equals C, then spiro might make you more susceptible to COVID-19.

The problem here is that no matter how scary that risk sounds, its still theoretical. Nobody has conclusively proven that taking spiro, or any other medication that affects our ACE2 receptors, will make us more likely to get infected. And given how long it would take to conduct a study and produce any credible evidence one way or the other, nobody will be able to conclusively prove it any time soon.

On March 17, the American Heart Association, the American College of Cardiology, and the Heart Failure Society of America released a joint statement addressing concerns about ACE2 receptor-interactant medications and COVID-19 risk, telling patients to continue taking ACE inhibitors and angiotensin-receptor blockers (ARBs) as prescribed. Radix of Callen-Lorde told VICE that trans people currently taking spiro should continue to do so, as well. Rather than worry about a theoretical risk, he recommended that we instead focus our attention on the very real health risks trans people will face in this pandemic.

Trans people have always had inadequate healthcare and are generally discriminated against in our systems, all of which will continue, he said. Imagine if you were a trans person whod had negative healthcare experiences, and you developed COVID-19. Youre probably sitting at home and worrying about whether you should even go to the emergency room. Those are the issues we should be concerned about: encouraging trans people to seek out healthcare if they need it.

Thats not theoretical. Thats real.

UFC president, Dana White is a polarizing figure. Hes done a tremendous amount for the UFC and the sport of MMA in general. Yet the way he conducts himself, some of his comments and antics, rubs people up the wrong way. But Dana White is Dana White and hes unapologetic about that fact. Hes achieved a great deal of success to date, so he isnt about to simmer down.

Cross Dana White at your peril. A lot of UFC fighters havent heeded this advice. It doesnt make any sense to bite the hand that feeds you. But some fighters think theyre entitled, think theyre the real deal, and take issue with some of Danas comments and practices. Whatever the reasons may be, these 16 fighters have come under fire from the UFC president, have felt Dana Whites wrath. Some are out of the doghouse, but others on this list have remained in Danas bad books. These are 16 fighters whove found themselves in Dana Whites doghouse.

The beef between these two supposedly started due to contractual issues. Randy announced hed be moving on from the UFC because he wanted a fight with Fedor Emelianenko. But Dana thought he had different motives, that he wanted to get involved with Hollywood and was meeting someone from the industry that Dana had issues with in the past.

Unsavoury things happen in the Octagon from time to time. But fighters still have to keep their cool, have got to maintain their professionalism. Paul Daley ended up in the doghouse after he sucker-punched an opponent way after the bell. Dana said that Paul was done, that he was finished with the UFC, and regardless of how good he was, he wasnt coming back.

The Tito Ortiz- Dana White feud is perhaps one of the biggest involving the UFC president. Theyve had a longstanding rivalry and part of the reason for this is because Titos been very outspoken regarding the treatment of fighters by the UFC. He was in Danas doghouse, and Dana was even set to release him at one point because a fight between the two was on the cards!

Lyoto ended up in the doghouse when he accepted a fight at short notice, then demanded more money because of it. This no-doubt angered Dana who refused to shell out the cash and adhere to Lyotos demands. Lyoto didnt get the cash and he didnt get the fight. Dana ended up going elsewhere.

Way back in 2002, Josh Barnett was a UFC Champion. But his career was soon over. Josh tested positive for performance enhancers three times. When giving interviews on the matter, he seemed very blas, refused to acknowledge what hed done and denied it every time. Dana White was incensed and soon let Josh Barnett go.

Former Pride 205 and 185-pound champion Dan Henderson landed up in Dana Whites doghouse for quite some time. Once upon a time there were talks of Dan resigning for the UFC. Dana White admits pushing him to sign for Strikeforce. In a way, he forced his hand. He just didnt want to be associated with Dan. But he later resigned and headlined at UFC 139 and seemingly, all was forgotten.

Michael Bispings ludicrous actions at UFC 127 landed him in Danas doghouse and in plenty of hot water. The guy just lost it, and Dana subsequently lost it with Michael, and rightly so. Michael performed an illegal move on Jorge Rivera and then proceeded to spit on his opponents team in their corner.

Quinton Rampage Jackson went on something of a rampage. Hes got a big personality, as has Dana, and they frequently clashed. But Dana found it hard to forgive him when he was involved in a hit-and-run and subsequently led police on a high-speed chase. In time, Quinton was let out of Danas doghouse.

Although he was a champion and one of the greatest MMA fighters around, that didnt stop him from finding a place in Dana Whites doghouse. The two have been involved in one seriously bitter rivalry. Theyve been engaged in a war of words for many years. Dana hates his guts, thinks hes just generally a bad person.

Dana White has a tremendous amount of love and respect for Anderson Silva. Hes achieved great things in the Octagon. But Dana hasnt always been enamored by Andersons fighting style. At UFC 112, Dana actually got up and left during the fight because he deemed it to be an absolute snooze-fest. It marked a period where Anderson had gone off the boil a tad. Dana actually apologized to the fans for having to witness such a dire showdown.

Dennis Hallman was deemed to be a decent fighter, hence he was contracted with the UFC. But he had a bit of a whacky mind, certainly a crazy dress sense. His choice of attire for his fight at UFC 133 landed him in Danas doghouse. He wore really short shorts, almost a speedo that left little to the imagination. Dennis ended up getting knocked out, and Dana was incredibly happy about that as it meant Dennis didnt spend much time on television.

Brock Lesnars had his ups and downs, be it in wrestling or on the MMA scene. But hes a star performer, is always going to be the major attraction at any event. But that hasnt made him exempt from Danas doghouse. He found himself in there for a momentary period after bashing the UFCs sponsors. They had a whip the dog session, then moved on.

According to Dana White, dealing with Cris Cyborg has been a nightmare. He said theres always controversy surrounding Cris, just a build up of a lot of things. Dana isnt having to deal with Cris anymore though as she was recently let go and has signed a multi-fight deal with Bellator MMA.

Nate Marquardt was once on a course of testosterone replacement therapy. Thats all well and good, its totally legal. But a fighters testosterone levels have got to return to normal before they fight. Nate and his team didnt ensure this happened. Dana was disgusted and had to cut him from his scheduled fight, and subsequently, from the UFC.

Jon Jones has found himself in Dana Whites doghouse time and time again. After his recent misdemeanors, he may not be let out. Whether hes a champion or not, regardless of whether hes one of the worlds greatest MMA competitors, Dana can only put up with so much, might just chain that doghouse and throw away the key.

Conor McGregors a big, bolshie character. Its partly why hes one of the biggest MMA fighters, certainly one of the most popular, on the planet. But this personality has gotten him into hot water with Dana White on a few occasions. Hes committed driving offenses, there was that incident at Bellator 187, and not forgetting the bus attack at UFC 223 Media Day.

Fitness secrets I unveiled: On my journey to weight loss, I have realised that cardio gives you the best results when it comes to shedding those stubborn kilos. Sure, some diets work for some people, but theres a lot more that goes into losing weight than just eating a particular type of food or slashing your food intake in general. Many people struggle with this and theres no shame in that. There are even more people who dont want to give up the foods they love altogether. With Noom, they arent here to tell you youre never allowed to eat your favorite sweets. They are focused on the bigger picture.

If you are getting food from a restaurant, Noom does a good job of just having a general estimate of how much something like chicken marsala would register on the tracker, rather than breaking down each ingredient. You can also create custom dishes and save them, if they are going to be something you eat frequently, like a smoothie.

As the program progresses, Noom will also start offering exercises and activities you can do to help you stay fit. You can choose what kind of workout you want (cardio, weight training, etc.) and Noom will cater your workouts to your preference. THURSDAY, April 2, 2020 (HealthDay News) -- While you're hunkered down waiting for the coronavirus to abate, you might get inspired to lose weight. But which diet is best?

The short answer is that all diets seem to work. The long answer is you'll probably regain the weight within a year.

"There is no diet that somehow magically helps you keep the weight off," said Dr. Gordon Guyatt of McMaster University in Ontario, Canada, who led a review of more than 100 studies that looked at diets.

Follow any diet you like, he said, and you'll take off some weight and lower your blood pressure and cholesterol levels. That reduces your risk for heart disease, including heart attack and stroke -- as long as you keep the weight off.

But that's easier said than done, Guyatt said, and as the numbers on the scale inch up, the benefits in terms of heart disease risk disappear.

"If nothing else, that takes a lot of willpower," he said. "I'm sure there are some people who did keep the weight off, but on average, it is very difficult. So, sorry, no magic bullet."

For the study, Guyatt's team looked at 121 published studies, which included nearly 22,000 people who followed popular diets or a control diet.

These diets included 14 low-carbohydrate and low-fat diets, such as Atkins, DASH (dietary approaches to stop hypertension) and the Mediterranean diet.

Both the DASH and Mediterranean diets are rich in fruits, vegetables and whole grains, nuts, legumes and fish, and low in sugar, fat and red meat. They are often touted as heart-healthy, but Guyatt said it's not clear that they have been proven to reduce the risk of heart disease and stroke.

Over six months, people who followed low-carb and low-fat diets lost an average of nine to 11 pounds, and their blood pressure also came down, the investigators found.

But none of the diets improved levels of "good" cholesterol (HDL-C) or lowered levels of C reactive protein, which is tied to heart disease, the study authors noted.

After a year, people on all diets studied had regained weight and lost any heart-health benefit. Those on the Mediterranean diet, however, did maintain a small benefit in cholesterol.

Based on these findings, you can expect the same initial weight loss on any of the diets -- so which one you choose is really a matter of personal preference, the researchers said.

According to Samantha Heller, a nutritionist at New York University Langone Health in New York City, "Diets, by today's standards, do not work. By work, I mean a sustainable lifestyle that promotes a healthy weight and meets the nutrient needs of the individual." Heller was not involved with the new study, but reviewed the findings.

Fad diets are temporary, often very restrictive and not realistic, she said.

"The environment in which we live does not promote healthy living, and it is difficult for many to resist the temptations of convenience and fast food," Heller added.

A more plant-based diet of home-cooked and less highly processed foods -- in other words a Mediterranean-type approach -- can help people lower their risk for chronic disease, manage weight and boost energy, she said.

"We need to buckle down and face the fact that a constant diet of fast, junk and prepared foods is not healthy," Heller stressed.

And, beware: You can gain weight eating anything -- even healthy food -- so portions as well as physical activity matter, she warned.

"We may get complacent over time, start snacking a bit more, having slightly larger portions of food, be less active," Heller said. "And, over time, these small changes can add up to slow, weight gain."

The report was published online April 1 in the BMJ.
